Based within an integrated team of Physiotherapists and Occupational Therapists at Moor Lane Mills, Lancaster, you will be working with patients with a variety of conditions. The caseload comprises patients in their own homes, in Residential Care Homes and in Dolphinlee House, Lancaster - a 23 bed Intermediate Care Rehabilitation Unit.  There are both urgent and longer term rehab pathways within the service.

You will carry a caseload delegated by the nominated clinician, and be responsible for interventions with patients. You will carry out the assessment of an individual’s level of function, identifying those at risk of falls and how this risk can be reduced, maintain and improve mobility through exercise/advice and consider the persons activities  of daily living. Your intervention may include the provision of equipment. You will plan and modify treatment programmes and will work closely with family members / carers and other members of the local community health and social care services to meet the patients’ individual needs. You will also assist in the delivery of the weekly Falls Prevention Exercise and Information Groups that are run in Lancaster, Morecambe and Carnforth.

The aim of this service is to enable people to achieve their full rehabilitation potential following a fall/ injury or illness / to facilitate discharge from hospital and prevent unnecessary hospital admissions / to support people in the management of long term conditions and to promote good health & wellbeing and independence through therapy interventions.

Previous, significant working knowledge of Physiotherapy and /or Occupational Therapy is essential. You must work well as a team member, and be confident in sharing information (written and verbal) with colleagues, as well as demonstrating the ability to work independently. Applicants must have a genuine interest in people and be enthusiastic and motivated in enabling people to regain and retain their independence.

You will be supported by experienced senior clinicians and a team leader. Continuing professional development is actively supported via clinical supervision/caseload management, Appraisal/Personal Development Reviews and competency based in-service training.

The service is currently provided to people who live in North Lancashire and are registered with a Morecambe Bay CCG GP practice, and therefore the post holder would need their own transport to enable them to visit people in their own homes.

The service is currently commissioned Monday – Friday 0830 – 1700.

As part of the Community Healthcare Services working in North Lancashire, we work in partnership with Bay Health & Care Partners to support the Sustainability and Transformation Plan for Morecambe Bay, developing an Integrated Health and Care System, commissioned by Morecambe Bay Clinical Commissioning Group.

We continue to explore new and innovative ways of working, through integration and alignment to the Integrated Care Community structures and Primary Care Networks being developed by groups of local GP practices. We support staff in the delivery of high quality, safe and effective patient care.
