We are looking for an enthusiastic and motivated individual who can work autonomously, and as part of the corporate cancer team, to lead the roll out of personalised care for cancer patients.

The Right By You Project Manager will implement the Personalised Care agenda. This will ensure cancer patients receive Holistic Needs Assessments (HNA’s) so that individual needs and concerns can be identified and addressed. In addition, the post holder will support the roll-out of Treatment Summaries so that health and wellbeing can be optimised including how to seek help if new symptoms arise. A large part of the role will be to support the implementation of new follow-up pathways for patients diagnosed with cancer. This will involve working closely with the clinical teams to aid with the transition from traditional follow-up pathways to self-supported management follow up (SSMFU) remote surveillance pathways

This strategic aim for Macmillan is closely aligned with the NHS Long Term Plan and the ambition to deliver Personalised care from point of diagnosis including supported self-managed follow up.

The post holder will work the clinical nurse specialists and Macmillan support worker workforce and the wider cancer workforce to adopt the principles of personalised care. The role will work also closely with the Service User Involvement Lead to ensure the voice of the person living with cancer is represented.

We are looking for an enthusiastic, self-motivated person with a pleasant manner and good inter-personal and organisational skills with a “can do” attitude and a passion to improve experience for cancer patients.

The post is based at the Royal Lancaster Infirmary, with travel throughout Morecambe Bay.
