An exiting opportunity has become available for a Band 4 Patient Navigator within the Medicine Care Group.

Main duties of the job

Working within a speciality team to manage the patient pathway taking into consideration all aspects of that pathway to include: - NHS national performance targets, RTT, diagnostics and cancer.
Monitor all patients on awaiting results in line with IRD ensuring they are managed appropriately - ensure all patients receive appointments in a timely fashion - Ensure all patients outcomes from clinical appointments are actioned.


Working for our organisation

The post holder will work with other Patient Navigators (PN) and the Assistant service manager.
The post holder will deputise for the assistant service managers to ensure the day to day supervision of secretaries and clerical officers within the speciality admin teams, escalating any concerns or issues to the Administration Manager.
The post holder will take an active role in ensuring all secretarial/clerical staff are up to date with TMS and ESR.
It is essential that the post holder exercise initiative commensurate with the role and that confidentiality is maintained at all times.
