Job overview

An exciting opportunity has arisen for a Band 6 District Nurse who already holds the Specialist Practitioner qualification, to come and join our team at Ulverston within Ulverston & Dalton District Nurses.

If you are innovative and passionate about quality patient care then we are the team for you and we would love you to be a part of this busy, very supportive and dynamic team. We cover from 8:30 am till 17:00 offering a 7 day service and work closely with our multidisciplinary team colleagues providing all aspects of care from wound care, palliative care to management of long term conditions. Our area covers Ulverston and the villages surrounding it, including Greenodd, Spark Bridge, Oldingham and Swarthmoor.

Main duties of the job

The post holder role will be required to:

 Support the Team Lead in providing clinical leadership and management of the clinical caseload for the team.

 Be responsible for the allocation, assessment, planning, implementation and evaluation of programmes of care without direct supervision.

 Demonstrate procedures to and supervise unqualified and qualified staff.

 Maintain and ensure high standards and continuity of care on a 24 hour basis.

 To support the movement of patients between the communities in-patient beds, secondary care and community teams where appropriate.

The majority of visits in the community will be undertaken by the post holder on their own and unsupervised. You will be expected to manage your own time and delegated caseload of higher complexity patients and to oversee the visits of other team members.

Working for our organisation

The Post Holder will be required to work as an integral part of the Community Nursing Team; flexible working will be required to meet the needs of the patients and will hold 24 hour delegated responsibility for the team caseload. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.

  • Are educated to RGN level 2
  • Have a District Nursing Specialist Practitioner qualification or currently undertaking the course
  • Are able to demonstrate a commitment to high quality care and service provision
  • Have the ability to work flexibly and as part of a team
  • Have the ability to assess and formulate care plans
  • Are able to teach and support student Nurses and Health Care Assistants
  • Practice Assessor / Practice Supervisor for student nurses
  • Have enhanced communication and interpersonal skills
